The admission form is the first record a school keeps about a child, and every later record — fee, attendance, result, leaving certificate — depends on it being right. A good form is short enough that parents fill it completely, and complete enough that the office never has to call them back. Below is a field-by-field checklist, a sample layout and a list of documents to collect.

Documents to collect
- Copy of the child's B-Form
- Copy of the father's or guardian's CNIC
- Recent passport-size photographs (usually two to four)
- School leaving certificate from the previous school (for classes above the entry class)
- Last result card or progress report
Tips
- Ask for the name exactly as on the B-Form. It will appear on certificates and board registration later; spelling differences cause trouble years afterwards.
- Keep the form to two pages. Anything you won't use, don't ask for.
- Mark one mobile number for school messages. Most missed notices go to a number nobody checks.
- Protect what you collect. CNIC copies and health details are personal data — keep them in a locked cabinet or a system with access control, not a shared folder.
